Agony is a status effect that happens when injured for longer than 4 days.
The ways to get hurt are by Trimming Ted's beard wrong, getting hurt on expeditions, Injured by self-diagnosed injuries, Mary Jane's Diary Event where you read diary, Raiders Hurting You, and Going in Scientist Experiment/Agent Bunker event and that's about it :). Then, simply wait for about 5-6 days on Reatomized. If you're on the 60 Seconds original, it takes 10 days.
This status effect only happens in 60 Seconds; in 60 Parsecs!, if you're injured for longer than 4 days, your health status becomes weak and you will eventually die.
